Insurance
As a licensed professional counselor with a doctorate degree, I find that sessions with clients are almost always covered at least in part by their health insurance or employee benefit plan. I give clients the paperwork needed to submit for reimbursement on a monthly basis.
It can be helpful to call your insurance company to determine:
Deductible for mental health coverage
Reimbursement rate
Number of outpatients sessions covered per year
If accessing benefits, please keep in mind I would be required to give you a mental health diagnosis (even though this is not the lens through which I see anyone).
New Client Forms
1. Download the New Client Form
2. If you are coming for Couples Counseling, please download the Relationship Information and Preparation Form and it bring to our first session (or email it to me if we are meeting virtually).
3. If you are coming for Family Business or Coworker Counseling, please download the Family Business Relationship and Preparation Form and it bring to our first session (or email it to me if we are meeting virtually).
4. If you are coming for Family, In Law, or Sibling Counseling, please download the Family-Sibling Relationship Form and bring to our first session (or email it to me if we are meeting virtually).
Policies
I totally understand that unanticipated events happen occasionally in everyone’s life. In my desire to be effective and fair to all clients, the following policies are honored:
Cancellation
24 hour advance notice is required when canceling an appointment. This allows the opportunity for someone else to schedule an appointment. If you are unable to give 24 hours advance notice you will be charged the full amount of your appointment.
No-shows
Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show.” They will be charged for their “missed” appointment. You may, however, send someone else in your place.
Late Arrivals
If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours.
